주요 업무 (Responsibilities)
신체적, 인지적, 심리사회적 평가를 포함한 노인 종합 사정 수행 및 개인 맞춤형 간호 계획 수립
의사, 치료사, 사회복지사 등 다양한 전문가와 협력하여 다학제 팀(IDT) 기반의 통합 케어 조정
만성 질환 모니터링 및 관리, 건강 상태 변화의 조기 발견과 입원 예방을 위한 신속한 중재 수행
약물 투여, 치료 처치, 상처 관리 등 직접 간호 제공
가정 방문을 통해 안전 환경, 복약 순응도, 지원 체계 점검 및 의료 장비 사용 적절성 평가
담당 환자의 케이스 매니저 역할 수행 및 병원·재활·가정 간 전원 과정 관리로 연속적인 치료 보장
환자 및 보호자에게 질병 관리, 예방 치료, 건강 유지 방법 교육 제공
규정, CMS 가이드라인 및 품질 개선 기준에 따른 정확한 문서화 및 기록 유지
자격 요건 (Education & Skills)
캘리포니아 간호위원회(California Board of Registered Nursing) 발급 등록 간호사(RN) 면허 필수
미국심장협회 (American Heart Association) 발급 Basic Life Support (BLS) 자격증 (CPR & AED 포함)
한국어 능통 필수

Responsible for providing care for the physical, mental, and emotional well being of the elderly population at the center with the collaboration of other members of the IDT Team. Supervises MA, CNA, LVN, and other members of the clinic team, and often collaborates with the Nurse Practitioner to provide the best care for the participants.
Responsibilities:
Conduct comprehensive geriatric assessments (physical, cognitive, psychosocial) and develop individualized care plans for participants
Coordinate care as part of the interdisciplinary team (IDT), collaborating with providers, therapists, social workers, and other disciplines
Monitor and manage chronic conditions, identifying changes in health status and implementing timely interventions to prevent hospitalizations
Provide direct nursing care, including medication administration, treatments, and wound care as needed
Perform home visits to assess safety, medication adherence, and support systems, and ensure appropriate use of medical equipment
Serve as case manager for assigned participants, overseeing care transitions (hospital, rehab, home) and ensuring continuity of care
Educate participants and caregivers on disease management, preventive care, and health maintenance to support independence
Maintain accurate documentation and ensure compliance with regulations, CMS guidelines, and quality improvement standards
Education and Skills:
A license to practice as a Registered Nurse issued by the California Board of Registered Nursing
Current Basic Life Support (BLS) for Healthcare Providers (CPR & AED) Program certification issued by the American Heart Association
Fluent in Korean
Geriatric experience preferred
Compensation
$45-55/hour
